Skin 101, Episode 3: The Hypodermis

Clients ask me constantly why their face looks different at 45 than it did at 25, even when their skin is genuinely healthy — no major sun damage, good home care, the works. Most of the answer isn’t happening in the epidermis or the dermis at all. It’s happening underneath both of them, in a layer almost nobody thinks about: the hypodermis.

We’ve covered the epidermis and the dermis in this series. This last piece is the one that explains a lot of what people call “aging” but is really just structural change happening below the surface.

The Hypodermis: The Foundation Underneath It All

The hypodermis, sometimes called the subcutaneous layer, sits beneath the dermis and is made up mostly of fat and connective tissue. Its job is straightforward: cushion, insulate, and store energy. But it also does something people rarely connect to skin — it gives the face its youthful volume and shape.

This layer is organized into distinct fat pads across the face — in the cheeks, temples, and under the eyes. In youth, those pads sit plump and in place. Over time, they naturally shrink and shift downward, which is a huge part of why a face can look “older” even when the skin itself is in great condition. It’s not that the skin failed. It’s that the foundation underneath it changed shape.

Why This Layer Gets Overlooked

Most skincare conversations stay focused on the epidermis and dermis, because that’s where products and topical treatments can actually reach. The hypodermis is deeper, and nothing you put on your skin touches it directly. But everything happening in the layers above it is still connected to what’s happening below — healthy circulation and lymphatic flow through the whole system support the fat and connective tissue in this layer too, not just the surface.

How This Shows Up in the Treatment Room

This is where lymphatic drainage earns its place as one of my favorite treatments, not just for how relaxing it is. Gentle, rhythmic movement supports the body’s natural waste-removal system through all the layers, reducing puffiness and helping the tissue underneath function the way it’s supposed to.

Microcurrent works this layer differently — low-level current acts almost like a workout for the facial muscles that sit just beneath the hypodermis, improving tone and circulation in a way that supports the structure above it. And because this is a layer that changes slowly, over years, it’s exactly why I’ve always said consistency matters more than any single treatment. This isn’t a layer you influence overnight, in one visit, or with one product.

The Takeaway

Understanding the hypodermis changes how I think about aging skin — and how I want my clients to think about it too. A lot of what people chase with “anti-aging” products is actually a structural, foundational change happening in a layer skincare alone was never going to reach. That’s not a reason to give up on your skin. It’s a reason to support it consistently, at every layer, for years.

That’s the whole point of this series. Skin isn’t one thing — it’s three layers working together, each with its own job, each responding to different kinds of care. Once you understand what’s actually happening underneath the surface, skincare stops feeling like guesswork.
